Failed drug test not on my dac report. But license were suspended.

I applied for a trucking company in October 2014 and I failed a pre-employment drug test but they did not report it on my dac report. and I never worked for the so I don't use them as an pre employer on any applications I fill out. Do I have to put on an application that I have a failed drug test. I did get my license suspended for 30 days in NC because state law require the lab to turn it in.
