Slip trip and skip, admission to school...

I had a fall accident twenty-four years ago. I have since fully recovered from a leg injury and a mild head injury. In light of that, would a trucking school accept me?

And if so, how can I get a doctor's note subsequent from the physical examination? Which is what the clinic was asking me for but I am pretty sure that information is no longer available...
